Follow the steps below to make your transaction as easy as 1-2-3.
Step 1
Select your transaction type and download all the necessary documents.
To best serve our customers, please have the required documents completed in advance. For your convenience, we can begin your IRP/IFTA transaction via email. Originals must be submitted in-person or via mail.

- IRP/IFTA Application Form 70-0508
Physical address of business must be in Arizona. - FRP Apportioned Registration Application Form 70-0502
- New Applications must be completed entirely.
- Adding vehicles or making changes, complete Schedule C.
- Actual miles reported, complete Schedule B.
- USDOT Number If you do not have a USDOT Number you MUST apply for one online at fmcsa.dot.gov.
- Federal Heavy Weight Vehicle Use Tax Form (2290)
For vehicles greater than 55,000 lbs. Confirmation records from the IRS are acceptable. - Proof of Ownership for each vehicle. If you have not yet started the title process, see the instructions at the end of page 2.

Required if the owner of the vehicle is someone other than the owner of the IRP account. - Power of Attorney – Motor Carrier and Tax Services Form 96-0441
Required if signing the application on applicant’s behalf. - Record Keeping Agreement Form 96-0537
Must be signed by the licensee or an authorized officer of the business. NO Power of Attorney of agent’s signature will be accepted on this form. - Proof of Address Applicant is to provide One proof of Established Place of Business and Three of the Established Proof of Residence. An Established place of business is a physical structure that is open for business and staffed. An Established place of Residence means where you live, where you own or rent property.

- Real-estate ownership document
- Real-estate lease agreement (cannot be a short-term lease)
- Rent or mortgage payment receipt.
- Current Utility bill
- Current Arizona driver’s license (if applicant is an individual)
- Paid personal income taxes to Arizona.
- Copy of the Filed Federal Income Tax return
- Other factors with clear evidence of the Applicant’s legal Business or Residence address

- Article of Incorporation in an Active Status, principal owner listed is a resident of Arizona.
- A vehicle title in Arizona in the business name or principal owner name with residence address

- Bond Requirements will be required if you are an IFTA applicant and any of the following apply to you:
- Not based in a jurisdiction that is a member of IFTA.
- Do not have a good standing status from another member jurisdiction as a result of a previous license.
- Do not have a verifiable history as a motor carrier in Arizona or any other member jurisdiction.

- Original Title/MCO or copy of out-of-state title (for ARO)
- Power of Attorney – Motor Carrier, Form 96-0441 (when applicable)
- Lessor Authorization, Form 40-0207 (when applicable)

IRP RENEWAL
- FRP Apportioned Registration Application Form 70-0502
An additional FRP Apportioned Registration Application will be needed to indicate changes when adding, amending, or deleting vehicles during renewal. Complete Renewal Application Entirely. - Federal Heavy Weight Vehicle Use Tax Form (2290)
For vehicles greater than 55,000 lbs. Confirmation records from the IRS are acceptable. *May be required.* - Alternative Fuel Certificate Form 48-1303 needed for any vehicles fueled by alternative fuel provided ONLY by ADEQ.
- Power of Attorney – Motor Carrier and Tax Services Form 96-0441
Required if signing the application on applicant’s behalf.
IFTA RENEWAL
- IFTA License Renewal Application Form 96-0424
- Record Keeping Agreement Form 96-0537
Signed by the licensee or an authorized officer of the business. NO Power of Attorney of agent’s signature will be accepted on this form. - Accounts that do not have a valid email address, no email address, or if the account is not in compliance, will have to submit an IFTA Renewal Form with changes requested to update the account.
- $10 filing fee will be applied.

- Comments section may also be completed to notate what the carrier is requesting. Example: Replacement Plate
- Copy of current IRP Apportioned Cab Card for specified vehicle.

Step 2
NOTICE: Our office is temporarily closed. In the meantime, we invite you to submit your forms via email.
Bring your documents into our office or for your convenience, we can begin your IRP/IFTA transaction via email. Originals must be submitted in-person or via mail.
If you’re emailing the documents, please be aware that processing may take 3-5 business days. Processing times tend to be greatest during the beginning and end of the month. Once your documents are processed, a bill will be emailed to you.

Step 3
Once your application has been billed, make your payment and provide a label to ship.
You can make your payment by money order, cashier check, Com-check, T-check or EFS-check (Must be made payable to Auto Tag of America). Credit cards can be used with an additional fee of 3.25% of the total bill. We are unable to accept Business and Personal checks.
